NCCN and ASCO guidelines support bisphosphonate use in multiple myeloma

National Comprehensive Cancer Network (NCCN) guidelines16

  • Bisphosphonate therapy should be given to all patients with documented bone disease, including osteopenia

American Society of Clinical Oncology (ASCO) guidelines17

  • Initiation of therapy
    • — ZOMETA (4 mg via 15-minute infusion) or pamidronate (90 mg via at least a 2-hour infusion) every 3 to 4 weeks is recommended for patients with evidence of osteolytic lesions
    • — For patients with pain secondary to bone metastases, ZOMETA or IV pamidronate should be administered as recommended above
  • Duration of therapy
    • — Therapy should be continued for 2 years and thereafter at the discretion of the
      treating physician
